Split them out and the price picture changes. For a mechanical ventilator, the upfront cost is the smallest part of the story; maintenance, alarm management, training and disposable circuits can cost more over five years. Healing caps, temporary abutments and screws are another line item that looks small until you multiply it by 300 cases. And shipping is the one that still bothers me. Every spreadsheet analysis said switch to a lower-cost distributor. My gut said their lead time was too risky. We switched anyway, and within one quarter we paid $1,150 in rush fees to cover the gaps. The savings evaporated.
